Patient information

Your visit, insurance
& common questions.

A little preparation, a lot more clarity. Find the practical information you need before arranging care.

Your first steps

Start with the appropriate contact pathway. A visit can be arranged only after the practice confirms that the location, clinical needs, and scheduling requirements are a fit.

  • Before a visitClinical needs, location, insurance, and any required referrals or authorizations are reviewed.
  • During an evaluationThe physician assesses the wound, reviews relevant history, and discusses an individualized plan.
  • After an evaluationFollow-up and care coordination depend on the findings and the agreed treatment plan.

Insurance & costs

Before scheduling, ask the practice and your insurer to clarify these three things. A visit request is not a guarantee of coverage.

  • Your specific planIs Prime Heal participating in your plan, and does the plan cover the proposed visit and care setting? Have the exact plan name available when you call.
  • Referrals and authorizationsDoes the visit or a proposed treatment need a referral or prior authorization, and who is responsible for arranging it?
  • Your expected costsWhat deductible, copay, coinsurance, or other costs may apply? Ask whether dressings, supplies, devices, or outside services are billed separately.

Plan participation and payment options are confirmed directly with the practice. No list of accepted plans is published here until it has been verified.

Already receiving care?

For appointment changes or routine questions, use your established care-team contact. You do not need to start a new-patient request. Keep your dressing instructions, supply arrangements, and follow-up plan available.

Common questions

14 questions

Where do visits take place?
Visits are arranged in suitable homes and residential care settings, including nursing and rehabilitation facilities. There is no walk-in office.
How do I know whether mobile care is appropriate?
The practice must review clinical needs, location, and availability before arranging a visit. Some wounds or procedures require another setting. A website inquiry cannot determine eligibility.
Can I request care without a clinician’s referral?
You can contact Prime Heal directly for yourself or a loved one about wound or skin-injury care. Your insurance may still require a clinician’s referral or prior authorization before a visit or treatment. Contacting the practice does not confirm eligibility, coverage, or an appointment.
Which insurance plans are accepted?
Call with your insurance plan name so the practice can confirm participation, benefits, and any referral or authorization requirements before scheduling.
Will there be an out-of-pocket cost?
That depends on the plan, benefits, and services. Ask about deductibles, copayments, coinsurance, and any other expected charges before care is arranged. Verification does not guarantee payment by an insurer.
Can my caregiver speak with the practice?
A caregiver may help arrange care. The practice will confirm consent or other appropriate authorization before discussing protected patient information.
Do I have to stop seeing my current care team?
The mobile-care model emphasizes coordination with existing clinicians and home-health or facility staff. The roles of each team should be clarified when care is arranged.
Will every wound need the same treatment?
No. Clinical findings, the cause of the wound, overall health, and the care setting guide an individualized plan. A listed condition does not imply a particular procedure or outcome.
Can wound-vac therapy be managed at home?
Some prescribed systems can be used at home, but the wound, device instructions, dressing-change support, supplies, monitoring, and caregiver training must all be considered. Availability must be confirmed with the practice.
Do all patients need advanced wound products?
No. The care plan begins with the wound’s cause and foundational care. Additional devices or products require individual review and are not automatically appropriate, available, or covered.
Can I upload a wound photograph?
Please ask the practice for an approved secure channel if wound photographs or records are needed. Do not send them to the general email address.
When will the practice respond?
Call the practice to discuss availability and when to expect a response. For urgent concerns, contact your treating clinician; for a medical emergency, call 911.
I am an existing patient. What should I do?
Use the contact information and instructions already provided by your care team, or call the practice for help.
Is a new request a confirmed appointment?
No. Clinical suitability, location, coverage requirements, and availability must be reviewed, and an appointment must be confirmed directly.
