No clinical information is collected through site forms
Blank patient forms are available as PDF downloads. This site does not receive completed forms, file uploads, patient accounts, or clinical submissions. Fill and save a downloaded PDF on your own device, and bring it to your visit or call for an approved secure return method. Contact selections change which instructions appear; they do not send a request or record a patient’s information.
No marketing trackers have been added
The site application does not include advertising pixels, session recording, or analytics scripts, and it does not use browser cookies or local storage. The hosting and access-control service may process technical information needed to operate and protect the site.
Email links
Email links open your email application for general and business inquiries. This website does not send or store those messages. Do not email patient details, medical records, or wound photographs; ask the practice for an approved secure channel.
External resources
Links to patient education resources leave this site. Those organizations have their own privacy practices. Do not include patient information in a website address or search query.
Clinical privacy is separate
A website privacy statement does not replace the practice’s HIPAA Notice of Privacy Practices. The practice’s Notice of Privacy Practices, effective October 11, 2026, is available on the patient privacy page. The privacy contact is Karapet Davtyan, MD, CWSP.
Before public release
The final policy must reflect the actual practice contact details, hosting arrangements, approved intake vendors, information flows, and any future measurement tools. This review-page statement describes only the current site application.
