New Patient Forms

A short, three-page packet. Answer what you can—we will work through the rest together.

Start with these three pages

The packet covers your contact and insurance details, a brief health and wound history, and consent and communication choices. Bring your insurance cards and current medicine list instead of copying every detail.

  1. Download first. Open the saved PDF in a PDF reader to type into the fields, or print it and write by hand. Some phone previews do not save entries reliably.
  2. Complete what you can. It is okay to leave uncertain answers for the visit. Review the choices on page 3, then print and sign the applicable signature lines.
  3. Bring it to your visit. Keep a copy for yourself. If you need to return it ahead of time, call us for an approved secure method.
Keep your information private. This website does not accept completed forms or uploads. Please do not email completed forms, medical records, or wound photographs to the general inbox. Downloading or completing a form does not schedule a visit.

Privacy notice — read and keep

Our Notice of Privacy Practices explains how health information is used, your privacy rights, and how to contact us. It is separate from the short packet and has no patient fields to complete.

Additional forms — only if requested

You do not need to complete these for every first visit. The care team will tell you whether a page is needed.

When would I need an additional form?
  • Page 1: Records authorization. For a specific release of records when an authorization is needed.
  • Page 2: Representative information. When a parent, guardian, or another legally authorized person acts for a patient.
  • Page 3: Procedure consent. Complete only with the treating clinician after discussing the specific procedure. Do not sign an incomplete form.
